I am a Nurse Manager in the long-term care sector overseeing all departments associated with the Dementia unit, ensuring that residents physical, psychological, spiritual and environmental needs are met. Through my work I continue to advocate for residents and their loved ones. My focus is on optimizing the quality of life and quality of care of my patients, which I understand that their care should be geared towards a comfort model approach to prevent suffering and distress associated with chronic illness. Understanding this, wellness begins from the time of
admission.
My goal is to assist you in completing your P.R.I. for a smooth admission in to a health care facility for long term, short term or respite care.
ADMISSION PROCESS
The process of admission to facilities requires a completed Patient Review Instrument (P.R.I.). This document was developed by the Dept. of Health and must be completed by a P.R.I. Certified Registered Nurse.
It is a summary of the patient’s current medical condition, demographics, physical and cognitive status
within the past 7 days.
I sincerely support your decision to have your loved one placed in a skilled care facility or rehab center
that will meet their needs. Together we can choose a facility that will best provide the care your loved
one requires.
My goal is to help you make a difficult process, less stressful. Your P.R.I. can be completed at your convenience and will be sent out to facilities of your choice.

*Please note:
-PRI is good for 30 days from the date of completion
-Fee is due prior to forms submission to facility
-Facility admission is a facility based decision upon their review
-PRI fee is non-refundable once forms are submitted to the facility
-Payment can be made via Zelle & CashApp

ï‚· Social Security Number
ï‚· Insurance: Medicaid/Medicare/Supplemental Insurance/Private insurance or Private Pay
ï‚· Medical Diagnoses
ï‚· List of all current medications, allergies and treatments
ï‚· Although not a requirement but if any Vaccinations received: Pneumonia (Pneumovac-23, Prevnar-13); Covid-19
ï‚· Not required but if there are any Advance Directives; Health Care Proxy or Power of Attorney
ï‚· We will discuss the full daily care needs (eating, movement, toileting) of your loved one

As a Nurse of over 30 years in the Long Term care sector, I believe health care should be driven by the
passion of ensuring the comfort of the patient and the support of their family. This is a service with the
aim of improving, maintaining and promoting the best quality of life for individuals. My goal is to
help headline a simple process of transferring your loved one to a facility for long or short-term care.
